Short-term Disability

Life sometimes doesn’t go as planned – so it’s important to be prepared! You never know when you may need to take time away from work to recover from an illness or injury, so Mission offers an STD benefit, just in case.

If you are absent from work due to illness or injury for more than 14 calendar days, you may be eligible for STD coverage.

There is a 14-day waiting period before you will start to receive this benefit. The maximum benefit period is 24 weeks, excluding the 14-day waiting period.

Mission covers the cost of replacing 40 percent of your pay during the benefit period.

You have the option to purchase additional coverage of 50 percent pay replacement. And, you may use PTO to increase your STD coverage to 100 percent pay replacement for the first two months of disability (including the 14-day waiting period).

Evidence of Insurability (EOI), or proof of good health, is required if you request to increase your coverage after your initial enrollment.

  • Go to Aflac.optisinsights.com/selfservice NOTE: First time users must register to create an account. You must enter MHS for the company name when prompted.

Premiums are paid with after-tax contributions (after applicable income taxes have been deducted), which means if you are paid a STD benefit, the money you receive will be tax-free.

Long-term Disability

If you are absent from work due to illness or injury beyond the time covered by STD (180 days), you may be eligible for LTD coverage.

Coverage is subject to the 3/12 pre-existing clause. (Benefits will not be paid for any disability which begins in the first 12 months of coverage which is due to a pre-existing condition unless there has not been treatment for that condition three months after your plan’s effective date.)

Evidence of Insurability (EOI), or proof of good health, is required if you request to increase your coverage after your initial enrollment.

Life, Accidental Death & Dismemberment (AD&D) Insurance

 Basic Life and AD&D

Mission Health automatically pays for Basic Life and Basic AD&D insurance for you. In the event of a claim, it pays one time your annual base pay*, up to a maximum of $600,000.

Supplemental Life and AD&D

You may also choose to purchase Supplemental Life and AD&D coverage in amounts of 1, 2, 3 or 4 times your annual base pay*, up to a maximum of $600,000. The cost of your supplemental life insurance is paid with pre-tax contributions through payroll deduction.

Evidence of Insurability (EOI), or proof of good health, is required if you request to increase your coverage after your initial enrollment during a future Annual Enrollment period. New or increased coverage will not be effective until your EOI application is approved.

Eligibility

I’m budgeted to work 20+ hours per week or 40+ hours per pay period and am not considered a PRN, contract or leased employee: You are eligible for coverage.

When you are age 70, your Life and AD&D insurance benefits will be reduced to 55% of the amount of your coverage. At age 75, your benefit will be reduced to 35.75% and to 27% at age 80.

*Annual base pay is defined as base pay excluding overtime and special pay, rounded to the next higher $1,000 if not already a multiple of $1,000.

Dependent Life Insurance

You can purchase additional coverage for your dependents: $30,000, $40,000 or $50,000 for your spouse, and $5,000 or $10,000 for your children.

Evidence of Insurability (EOI), or proof of good health, is required for all new enrollees with spouse coverage amounts of $40,000 or $50,000. You also must provide EOI if you want to add spouse life coverage after the first time your spouse is eligible for coverage. New or increased coverage will not be effective until your EOI application is approved. EOI is not required for child(ren) life coverage.

Long-term Care Insurance

This coverage provides for chronic care that may not be covered by other health insurance or Medicare. The flexible plan allows you to purchase the level of benefits that is right for you, and gives you the freedom to choose where to use them – whether a nursing home, assisted living facility, adult day care, or your own home.

Premiums are payroll deductible, and the coverage may also be available to your spouse, parents and other adults in your household.
